Role Overview

We are building a structured, technology-driven Family Health Command System focused on preventive care, medical coordination, and emergency preparedness.

We are seeking a proactive and experienced Nursing Professional with hospital experience who can support clinical coordination, patient monitoring, and structured health data management. This is not a routine hospital ward role. This is a structured, responsible, long-term healthcare coordination and monitoring position working within a technology-enabled health management system.

Key Responsibilities

1. Patient Care Support & Monitoring

Assist in monitoring patient vitals and health parameters.

Maintain updated patient health records and observations.

Track patient symptoms, medication adherence, and general health status.

Support continuity of care by maintaining organized patient information.

2. Medical Reports & Documentation Support

Review and organize patient lab reports, prescriptions, and discharge summaries.

Maintain structured digital medical records.

Identify and flag abnormal values or inconsistencies for further medical review.

Ensure proper documentation and record maintenance.

3. Care Coordination Support

Assist in coordinating consultations with doctors and specialists.

Track recommended tests, follow-ups, and medical appointments.

Prepare structured summaries of patient history and reports for review.

Ensure patients follow recommended care plans.

4. Preventive Health Monitoring

Maintain preventive screening schedules.

Monitor health indicators such as vitals, lab trends, and follow-up timelines.

Assist in tracking routine health check-ups and preventive care activities.

5. Emergency Preparedness Support

Maintain updated patient emergency information.

Assist in coordinating hospital support during urgent situations.

Ensure critical medical data is easily accessible when required.

6. Research & Healthcare Data Work

Assist in collecting and organizing structured health data.

Support research activities related to preventive healthcare and patient management.

Contribute to improving healthcare documentation systems and workflows.

Assist in reviewing healthcare processes for better patient management.

7. Technology & Platform Coordination

Work within a digital health management platform.

Maintain structured digital patient records and reports.

Assist in verifying and organizing simplified medical summaries.

Support improvement of digital clinical documentation processes.

Skills & Competencies
